Hospital discharge care is support at home in the days and weeks after a hospital stay, so recovery continues safely in familiar surroundings. We work with the discharge team, your family and community health professionals to agree a plan before you leave the ward, then provide the practical help that makes coming home manageable. Caring Care provides hospital discharge care across Walsall, Birmingham, Wolverhampton, Dudley, Sandwell and Staffordshire.
